Serves: Tulsa. Must be pregnant, have minor children residing permanently in the home, be disabled or handicapped, or be over the age of 55.
Documentation Required: Must provide one of the following for each member of the household: driver’s license, birth certificate, insurance card, passport, or Social Security card. Will need ID for family members.

Tuesday through Friday
9:00am – 2:00pm
Tuesday
4:30pm – 6:30pm
Third Saturday of each month
9:00am – 12:00pm
